We want your first visit to our office to go as smoothly as possible so that your family can relax and spend time getting to know our team. For your first appointment, we are happy to provide you links to access our new patient paperwork online. Here, you can download, print, and fill out our Health History form, and bring it with you to your child’s appointment.
Patients With Insurance Coverage
As a children’s dentist in Tucson, AZ, we want you to understand your child’s treatment as well as your financing for their treatment.

As a courtesy for patients with insurance coverage, we’ll verify your coverage and forward claims on to your insurance carrier. Estimated co-payments are due at the time of treatment. Insurance companies provide benefits according to your specific policy and coverage varies depending on the insurer. We always recommend being familiar with your individual insurance plan and understanding its benefits as well as its limitations. Additional tips:
- Please provide insurance information at the time of visit. Also provide any fee schedules your plan may follow because some insurance companies do not release that type of information.
- We’re happy to provide pre-treatment estimates upon request. However, your insurer should notify you the amount that will be covered.
- Please keep in mind that most insurance plans do not cover the entire treatment cost. We cannot guarantee what your insurance company will pay until our claim has been submitted. You will be held financially liable for all amounts left unpaid by your insurance company.
- Claims left unpaid over 60 days may become your responsibility.
Patients Without Insurance Coverage
Payment is due at the time of treatment. However, we’re happy to provide estimates to you prior to your appointment. Please remember, your child’s visit will be tailored to his/her individual needs. This means the treatment provided as well as any estimates may change on an individual basis as the appointment dictates.
We currently accept Visa and MasterCard. We generally do not extend credit, but ask that patients use external financing companies, such as Care Credit. In addition, Northwest Children’s Dentistry adds a $25.00 NSF fee for all returned checks.
Scheduled appointments are a confirmation that you will attend your visit. However, we supply appointment reminder cards, a phone call one week prior, and a phone call one business day before your scheduled visit. We understand that most families juggle a full and often hectic schedule, which means occasionally you’ll need to reschedule an appointment. Please call us at least 36 hours in advance if you need to reschedule/change your appointment. In addition, if you are more than 15 minutes late for your appointment, we may need to reschedule your visit for another day. We are here to serve you!
